Subscription Policy - Lower Alsace Township & Saint Lawrence Borough

• Lower Alsace Township and Saint Lawrence Borough utilize municipal taxes to purchase PREMIUM subscription policies on behalf of all residents of Lower Alsace Township and Saint Lawrence Borough. A “Resident” is classified as:

  • (a) Any and all residents that reside yearly in the household parcel residing in Lower Alsace Township or Saint Lawrence Borough limits including eligible dependents or;

  • (b) A college student that resides in a Lower Alsace Township or Saint Lawrence Borough household whenever class is out of session.

• Lower Alsace Township and Saint Lawrence Borough residents are not responsible for costs that are not paid by their insurance plan for "medically necessary" transportation as deemed by the insurer. Payment from any insurance source is considered payment in full. Services that are determined by the health insurer to not meet medical necessity will be charged the calendar year's CMS Fee For Services (FFS) Rate. Rate Information is provided HERE

Subscription Policy - Mount Penn Borough

• Mount Penn Borough utilizes municipal taxes to purchase BASIC subscription policies on behalf of all residents of Mount Penn Borough. A “Resident” is classified as:

  • (a) Any and all residents that reside yearly in a household parcel residing in Mount Penn Borough limits including eligible dependents or;

  • (b) A college student that resides in a Mount Penn Borough household whenever class is out of session.

• Unless an upgrade is purchased by the resident to a PREMIUM policy, Mount Penn Borough Residents are responsible for all fees associated with the cost of service including Co-pays, Co-Insurance, and deductibles. 

• Subscriptions only cover costs associated within the calendar year and expire at the end of the subscription period. Subscription fees can only be applied to incidents that occur after subscription has been postmarked or received through electronic methods. Requests for subscription applications to be retroactively applied to incurred debts will not be accepted.

• Mount Penn Borough Residents are awarded (3) THREE non-transport incidents within (1) ONE calendar year without incurring additional fees unless an upgrade is made to a PREMIUM plan which covers a total of (6) SIX non-transport incidents within (1) ONE calendar year. Purchasing additional subscriptions for the purpose of extending these thresholds will not be accepted.

• Subscription covers only services that are deemed "medically necessary" by your insurance provider. Services that are determined by the health insurer to not meet medical necessity will be charged the calendar year's CMS Fee For Services (FFS) Rate. Rate Information is provided HERE

Subscription Policy - Alsace Township

• Alsace Township does not currently utilize any form of municipal tax to fund subscription to Lower Alsace Ambulance Association. PREMIUM Subscriptions are offered for purchase to any resident that resides in a tax parcel of Alsace Township. A “Resident” is classified as:

  • (a) Any and all residents that reside yearly in a household parcel residing in Alsace Township limits including eligible dependents or;

  • (b) A college student that resides in an Alsace Township household whenever class is out of session.

• Only PREMIUM policies are offered to residents of Alsace Township.

• Subscriptions only cover costs associated within the calendar year and expire at the end of the subscription period. Subscription fees can only be applied to incidents that occur after subscription application has been postmarked or received through electronic methods. Requests for subscription applications to be retroactively applied to incurred debts will not be accepted.

• Alsace Township subscribers are awarded (6) SIX non-transport incidents within (1) ONE calendar year without incurring additional fees.
