Subscription Policy - Lower Alsace Township, Mount Penn Borough, & Saint Lawrence Borough

• Lower Alsace Township, Mount Penn Borough, and Saint Lawrence Borough utilize municipal taxes to purchase Subscription policies on behalf of all residents of of their respective municipalities. A “Resident” is classified as:

  • (a) Any and all residents that reside yearly in the household parcel residing in one of the above reference municipal limits including eligible dependents or;

  • (b) A college student that resides in an aforementioned municipal household parcel whenever class is out of session.

• The aforementioned residents are awarded (4) FOUR non-transport incidents within (1) ONE calendar year without incurring additional fees.

• The aforementioned residents are not responsible for costs that are not paid by their insurance plan for "medically necessary" transportation as deemed by the insurer. Payment from any insurance source is considered payment in full. Services that are determined by the health insurer to not meet medical necessity will be charged the calendar year's CMS Fee For Services (FFS) Rate. Rate Information is provided HERE

Subscription Policy - Alsace Township

• Alsace Township does not currently utilize any form of municipal tax to fund subscriptions to Lower Alsace Ambulance Association. Subscriptions are offered for purchase to any resident that resides in a tax parcel of Alsace Township. A “Resident” is classified as:

  • (a) Any and all residents that reside yearly in a household parcel residing in Alsace Township limits including eligible dependents or;

  • (b) A college student that resides in an Alsace Township household parcel whenever class is out of session.

• Subscriptions only cover costs associated within the calendar year and expire at the end of the subscription period. Subscription fees can only be applied to incidents that occur after subscription application has been postmarked or received through electronic methods. Requests for subscription applications to be retroactively applied to incurred debts will not be accepted.

• Alsace Township subscribers are awarded (4) FOUR non-transport incidents within (1) ONE calendar year without incurring additional fees.

• Purchasing additional subscriptions for the purpose of extending these thresholds will not be accepted.

• Subscription covers only services that are deemed "medically necessary" by your insurance provider. Services that are determined by the health insurer to not meet medical necessity will be charged the calendar year's CMS Fee For Services (FFS) Rate. Rate Information is provided HERE
