1. INTRODUCTION
Welcome to the Lower Alsace Volunteer Ambulance Association Scholarship Program. These Terms and Conditions outline the rules, requirements, and responsibilities associated with the Lower Alsace EMS Scholarship Fund. By applying for this scholarship, you agree to comply with the terms and conditions specified herein.

2. ELIGIBILITY CRITERIA
To qualify for the Lower Alsace EMS Scholarship Fund, applicants must meet the following criteria:
2.1 Residency: Applicants must be legal residents or citizens of the Commonwealth of Pennsylvania and attend and/or will graduate from the Antietam School District or will graduate from Exeter Township School District and resides within the municipal borders of the Borough of Saint Lawrence.
2.2 Academic Standing: Applicants must be currently enrolled in or have been accepted into an accredited institution of education.
2.3 Field of Study: The scholarship is available to prospective students and current students pursuing a degree or certification program in any of the following courses of study:
• Pre hospital Emergency Medical Services at the EMT-basic level or higher.
• Public Health Emergency Medicine
• Emergency Medical Services Administration
• Pursuant to section 3.2, a certification program or degree that describes an intention to pursue an education that will lead to the advancement of pre hospital emergency medical services.
2.4 Application Deadline: Applications must be submitted by May 31, of the calendar year. Late submissions will not be considered.
3. APPLICATION PROCESS
3.1 Submission: Applications must be submitted via the Lower Alsace Volunteer Ambulance Association Scholarship online portal at http://www.laems555.org/scholarship or sent to the following mailing address:

3.2 Required Documents: Each application must include:
• A completed application form.
• A personal statement or essay of no more than 2,000 words outlining the applicant’s goals, achievements, and why they deserve the scholarship.
• Proof of academic standing, such as recent transcripts or an acceptance letter from the educational institution.
• Two letters of recommendation from individuals who can attest to the applicant’s academic and/or personal achievements.
3.3 Application Fee: There is no fee to apply for this scholarship.

4. SELECTION PROCESS
4.1 Review: All applications will be reviewed by the Lower Alsace Volunteer Ambulance Association Scholarship Committee.
4.2 Criteria: The selection process will consider academic achievement, the quality of the personal statement or essay, letters of recommendation, and alignment with the scholarship’s objectives.
4.3 Notification: Scholarship recipients will be notified by June 30, of the same calendar year via Certified letter. If you do not receive a notification by this date, please assume your application was unsuccessful.
5. SCHOLARSHIP AWARD
5.1 Amount: The scholarship award is valued at $2,000 USD and is intended to support the educational expenses of student tuition. Any remaining balance may be applied to expenditures related to the delivery of education any must be approved by the Scholarship Committee.
5.2 Disbursement: The scholarship funds will be disbursed directly to the educational institution. The funds will be applied towards tuition, books, and other necessary educational expenses.
​5.3 Use of Funds: Scholarship funds must be used exclusively for educational purposes. Any funds used for non-educational expenses will require repayment to Lower Alsace Volunteer Ambulance Association.
5.4 Retroactive Award: Awards may be made retroactively to those meeting aforementioned criteria and has already completed/or are enrolled in a program consistent with § 2.3 of these Terms and Conditions.
6. TERMS OF AWARD
6.1 Progress Reports: Recipients may be required to submit periodic academic progress reports to Lower Alsace Volunteer Ambulance Association. These reports should include updated transcripts and a brief description of academic achievements and activities.

7. RESPONSIBILITIES
7.1 Accuracy: All information provided in the application must be accurate and complete. Providing false or misleading information will result in disqualification or termination of the scholarship.
7.2 Compliance: Recipients must comply with all scholarship terms and conditions and adhere to the policies of Lower Alsace Volunteer Ambulance Association.
8. TERMINATION AND REFUNDS
8.1 Termination: The scholarship may be terminated if:
• The recipient drops out of their program of study or changes their major to one not covered by the scholarship.
• The recipient violates any terms and conditions of the scholarship.
8.2 Refunds: In the event of termination, any unused scholarship funds must be refunded to Lower Alsace Volunteer Ambulance Association. The refund process must be completed within 30 days of the termination notice.
9. PRIVACY
9.1 Data Protection: Lower Alsace Volunteer Ambulance Association is committed to protecting your personal information. All personal data collected during the application process will be used solely for the purposes of scholarship administration and will be managed in accordance with Commonwealth of Pennsylvania data protection laws.
